Autism is a serious neurodevelopmental disorder that impairs a person's social and communication skills and can affect behaviour.

It is a part of the diversity of the human gene pool. In the United States, the cost of lifetime caring for one person with ASD is estimated to be $2.4 million. As Kathleen Seidel says, “it is as much a part of humanity as is the capacity to dream.” In India, cost of caring for a child with ASD is estimated at Rs 6,000 to Rs 25,000 per month. But persons of all ages and all symptom-severity-levels benefit from treatment. In adults, some ASD symptoms can overlap with symptoms of other mental health disorders, such as anxiety or attention-deficit/hyperactivity disorder (ADHD). And in some persons, it may not be diagnosed till they are adolescents or adults. Symptoms usually become apparent in children by the age of three. It is an in-depth conversation with the child’s caregiver and family members. India has about eighteen lakh persons, 1% to 1.5% children below the age of ten, and about 10% school-age children with ASD. And the symptoms may vary in severity. Autism is a serious neurodevelopmental disorder that impairs a person’s social and communication skills and can affect behaviour.

The overarching need of the hour is to develop a national programme on autism, to link researchers, clinicians, service providers to the end-users in the autism community in India. He is a Visiting Professor of Psychology at Ashoka University, India, and is Principal Research Adviser to the India Autism Center, Kolkata To this end, we recently conducted the first nationwide survey of research, intervention, and training priorities of the autism community in India. To this end, emerging lines of evidence suggest the feasibility of such an approach for both autism identification and intervention in an Indian context. While some of the more widely used tools have been translated and validated locally, recent years have seen a rise in the development of indigenous autism assessment tools. Two pillars for such an approach to succeed are the availability of a suitable non-specialist workforce (e.g. This difference is likely to be driven by a range of sociological factors, such as access to appropriate clinical expertise, the allowance of provisions for inclusion in mainstream schools, as well as availability of medical insurance coverage for autism interventions. Parallel efforts to widen the reach of diagnostic and intervention services through involving non-specialists, similar to a stepped-care model for psychological therapies, is required in order to bridge this chasm. We were looking for available records of the children who came to them, and were eventually given a diagnosis of autism. Instead of relying on existing medical records from hospitals, we translated and validated widely used autism assessment tools — and used these to estimate the prevalence of autism in nearly 12,000 schoolchildren. While biochemical pathways involved in core autism symptoms are unlikely to be different between cultures, there are notable cultural differences in who gets a clinical diagnosis of autism. Interestingly, none of the children who met the diagnostic criteria for autism spectrum in our study had a prior clinical diagnosis.

A lack of interest in wanting to learn and the usually typical joy of figuring out a new trick or skill appeared to be almost absent. The mountainous barrier that had to do with Noel’s motivation also had to be overcome. The transition then to the next level of what I call supreme joy was easy and took an unbelievably short time. There was such a difference in his disposition, motivation to learn, and in the understanding of others. I had to learn about his physical and motor difficulties precisely by observing him, noting every detail and then formulating methods to leverage his relatively strong points amidst the plethora of deficits. Surprisingly, the impact that emanated from the tennis wall practice began to radiate in every sphere of my life. When I was in a logjam with office politics or in financial upheaval, the person for me to run to was my boy, Noel. Following a whole-hearted and gracious acceptance of Noel the way he was, the path forward became truly unstoppable. I was able to reach a state of bliss when I was with Noel on the tennis courts. My persistent worry was that it was an enormous loss of time to learn to play. However, in the initial years, I was instantly caught in the harrowing turbulence of comparing Noel with other boys and girls his age that came to play. So, 15 years in the time scale would not be the same as it would be for a neurotypical person.

This inspired us to use songs to coach him in language skills, to describe situations he experienced and teach him the concept of one experience resembling another. Noel’s love for music soon segued into an obsession with the game of antakshari (with Bollywood songs) when a friend, who also became his free-hand exercise instructor, introduced him to it. The magnitude of its value in enhancing the child’s inner sense of self-worth and self-confidence and, in the long run, providing eternal companionship, according to me, is beyond measure. The unchanging melody structure, the same musical instruments, the same pauses and effects, the same images in sequence brought him a certain calm and became the sure-shot resource to turn to whenever he used to get stressed and a tantrum was brewing or, on a more serious note, approaching a meltdown. Noel could listen to a single song a hundred times in a loop or have the same CD run again and again in the car music system. Also, my hunch was that he liked that a particular song is sung the same way every time it plays and the visuals sync with the track which fed unfailingly into the autistic appetite for sameness. His first question would be ‘Do you have songs on your mobile phone?’ and if he had heard a song on that person’s mobile phone on an earlier occasion, then the next questions would follow: ‘Can I listen to Super Trouper by ABBA on your phone, please? He sang them all, from volumes 1-4, in the exact sequence as they appeared in the albums right through his childhood and adult years. It was astonishing and appeared to be a natural gift. When Noel was six, he started with Disney’s Children’s Favourites and they remained his favourites even when he turned twenty-six! I always pushed him to string together the reasons in his mind and express himself with whatever vocabulary he had. Accompanied by a courteous sales attendant, I embarked on the test drive, Noel in tow.
